Athletic Clearance
All Physical/Health Screenings need to be dated June 5, 2025 OR later to be used for the 2025-2026 school year.
CVHS Sports Physicals Night - June 5 at 4:30 PM in the small gym. Cost is $25 cash only.
The only paper form will be the physical/health screening form which is handed in to the Athletic Director.
