Athletic Clearance
CVHS Sports Physicals Night - Wednesday, May 27, 2026, at 4:30 PM in the small gym. Cost is $20.
Checks made payable to Shasta Orthopaedics & Sports Medicine or exact dollar amount ($20) for the physical.
Parents should be present as they need to sign paperwork, and if there are any issues, the doctors would want to speak with the parent.
The only paper form will be the physical/health screening form which is handed in to the Athletic Director.
